Choice of superannuation fund - standard choice form
For employees who wish to make a choice of fund. Give this form to your employee after you have completed Part A.

Direct debit request form
Through the Direct Debit Request (DDR) you are allowing us to debit amounts from your bank/financial institution account. The amount, how often and the date we will debit your account depends on what you instruct us to do.

Electronic funds transfer form
For use by employers to notify the Fund when a contribution payment has been made by EFT. This should accompany the contributions breakdown and be sent on the day of payment.

Employee schedule
In respect of each employee, complete the following details. Employer Sponsors, please use this form to tell us about the employees you are
enrolling in the Fund.

Employer application form
Application is made by the Employer to Farm Plan Pty Limited as Trustee of Prime Super. The Employer makes this application on behalf of those of its employees for whom they will contribute to the Fund. The application consists of this form and the initial contributions made on behalf of the employees. The application will be accepted and admission to the Fund will commence when the initial contributions are accepted by the Trustee.

Employer change of details
Used to update Employer details.
